Safety Outcomes of Brolucizumab in nAMDResearchers wrote that limited data exist on the real-world safety outcomes of patients with neovascular age-related macular degeneration treated with brolucizumab (Beovu, Novartis). They evaluated the real-world incidence of intraocular inflammation (IOI), including retinal vasculitis (RV) and/or retinal vascular occlusion (RO) for patients with neovascular age-related macular degeneration who underwent brolucizumab treatment, along with risk factors associated with these adverse events. Several of the researchers are employees of Beovu’s maker, Novartis, and others consult for the company. This cohort study included patients with nAMD in the Intelligent Research in Sight (IRIS) Registry and Komodo Healthcare Map. Patients initiating and receiving one or more brolucizumab injections from October 8, 2019, to June 5, 2020, with up to six months of follow-up were included. Main outcomes and measures included incidence of IOI, (including RV) and/or RO and RV and/or RO, and risk stratification for the identified risk factors. A total of 11,161 included eyes (from the IRIS Registry and Komodo Health database) with median follow-up times of 97 and 95 days were included. Here are some of the findings, respectively: Researchers determined that the incidence rate of IOI and/or RO was approximately 2.4 percent. Patient eyes with IOI and/or RO in the 12 months prior to first brolucizumab injection had the highest observed risk rate for IOI and/or RO in the early months after the first brolucizumab treatment. However, researchers added, given study limitations, the identified risk factors couldn’t be used as predictors of IOI and/or RO events, nor could causality with brolucizumab be determined. Delayed Intravitreal Anti-VEGF Therapy for Treating Retinal PathologyResearchers determined the anatomical consequences of delaying intravitreal injection (IVI) therapy with anti-vascular endothelial growth factor in patients using a treat-and-extend protocol. The retrospective medical record review included consecutive patients receiving intravitreal anti-VEGF therapy using a T&E protocol prior to and during the COVID-19 pandemic. The study included 923 eyes of 691 patients: 58.8 percent (543 eyes) had nvAMD; 25 percent (231 eyes) had DME; and 16.2 percent (149 eyes) had RVO. Mean patient age was 74.5 ±11.7 years. Here are some of the findings: Researchers wrote that delayed IVI treatment in eyes treated with a T&E protocol was associated with increased macular thickness and potential consequences for visual outcomes. Impact of Intra- and Subretinal Fluid on Vision Based on Volume Quantification in the HARBOR TrialInvestigators assessed functional associations of intra- and subretinal fluid volumes at baseline and after the loading dose, in addition to fluid changes after the first injection in patients with neovascular AMD who received an anti-VEGF treatment over 24 months. This was a post-hoc analysis of a Phase III, randomized, multicenter trial, in which ranibizumab was administered monthly or in a PRN regimen (HARBOR). Participants included study eyes of 1,094 treatment-naïve nAMD patients. Intraretinal (IRF) and subretinal (SRF) fluid volumes were segmented automatically on monthly SD-OCT images. Fluid volumes and changes were included as covariates into longitudinal mixed effects models, modeling BCVA trajectories. Main outcome measures included: BCVA estimates corresponding to baseline; follow-up and persistent IRF/SRF volumes following the loading dose; BCVA estimates of change in fluid volumes following the first injection; and marginal and conditional R2. Here are some of the findings: Investigators found that, while IRF consistently correlated with decreased function and recovery throughout therapy, SRF was associated with a more pronounced functional improvement. Correlation Between CST and VA Changes in nAMDInvestigators aimed to determine a correlation between changes in central subfield thickness and best-corrected visual acuity in neovascular age-related macular degeneration receiving anti-VEGF agents, as part of a post hoc analysis of VIEW 1 and 2 randomized clinical trials. This analysis included participants randomized: to ranibizumab 0.5 mg q4 weeks (Rq4); intravitreal aflibercept injection (IAI) 2 mg q4 weeks (2q4); and IAI q8 weeks after three monthly doses (2q8) to week 52, followed by capped pro re nata (at least q12 weeks) dosing to week 96. The relationship between changes in CST and BCVA was determined using Pearson correlation coefficient. Here are some of the findings: Investigators found weak or no correlation between changes in CST and BCVA with either agent or regimen, suggesting changes in CST shouldn’t be used as a surrogate for visual acuity outcomes in nAMD. Association of Flow Signals Within Polyps on OCTA with Treatment Responses After Combination Therapy for PCVResearchers evaluated the changes of blood circulation within the polypoidal lesions by OCT angiography in eyes with polypoidal choroidal vasculopathy after combination therapy with aflibercept and photodynamic therapy. A total of 46 eyes from 46 patients who underwent the combination therapy for PCV were followed for more than six months. OCT angiography covering an area of 6 × 6 mm2 including the macula were performed at baseline, two weeks, and three and six months post-treatment. Here are some of the findings:
Researchers wrote that persistent flow signals within polyps on OCTA at two weeks after combination therapy suggested less effectiveness of the initial treatment. Statin Use and Incidence of AMD: A Meta-AnalysisAge-related macular degeneration shares many of the same risk factors with atherosclerosis, and some researchers have postulated that lipid-lowering agents may help in preventing AMD. This meta-analysis investigated the possible role of statins in the prevention of AMD onset and progression. Scientists systematically searched MEDLINE, EMBASE, Cochrane CENTRAL and the reference lists of included studies from inception to September 2020. Studies were included if they measured the risk of AMD development or progression with statin use. The primary outcomes assessed were AMD incidence and progression. Secondary outcomes included the incidence of early AMD, late AMD, choroidal neovascularization and geographic atrophy. Here are some of the findings:

Intravitreal Anti-VEGF in DME: Review of Clinical Practice GuidelinesResearchers identified diabetic macular edema clinical practice guidelines that made anti-VEGF treatment recommendations, and assessed their reporting quality and their considerations when making recommendations. Sources of evidence included a sensitive search strategy in Embase, Google Scholar and hand-searching on 165 websites. Researchers extracted information from each CPG with a previously piloted sheet. Two independent authors applied the Appraisal of Guidelines, Research and Evaluation tool (AGREE-II) assessment for each CPG. Here are some of the findings: Researchers determined that most of the clinical practice guidelines that made recommendations of anti-VEGF for DME had poor quality of reporting, didn’t use systematic reviews and didn’t consider patients' values and preferences. New UWF Angiography Biomarker for Anti-VEGF Therapy in PDRResearchers quantified changes of the retinal vascular bed area (RVBA) on stereographically projected ultra-widefield (UWF) fluorescein angiography (FA) images in eyes with proliferative diabetic retinopathy following anti-vascular endothelial growth factor injection. The prospective, observational study included early-phase UWF FA images (Optos 200Tx) of 40 eyes with PDR and significant non-perfusion obtained at baseline and after six months. The images were stereographically projected by correcting peripheral distortion, and the global retinal vasculature on UWF FA was extracted for calculating RVBA by summing the real size (mm2) of all the pixels automatically. Here are some of the findings: Researchers wrote that RVBA appeared to be a new biomarker indicating efficiency of retinal vascular changes after anti-VEGF injection. RNFL Thickness as a Measure of Diabetic Retinal NeurodegenerationMarkers for clinical evaluation of structural changes caused by diabetic retinal neurodegeneration (DRN) haven’t been established. To study the potential role of peripapillary retinal nerve fiber layer (pRNFL) thickness as a marker for DRN, investigators evaluated the relationship between diabetes and glycemic control, regardless of diabetes status and pRNFL thickness. With data from a population-based cohort, they used general linear mixed models (GLMMs) with a random intercept for patients and eyes to assess the association between pRNFL thickness (via GDx) and demographics, and systemic and ocular parameters, adjusting for typical scan score. GLMMs were also used to determine the relationship between glycated hemoglobin/HbA1c (irrespective of diabetes diagnosis) and pRNFL thickness; diabetes and pRNFL thickness; and pRNFL quadrants affected in diabetics. A total of 7,076 participants were included. Here are some of the findings:
Investigators found that superior and inferior pRNFL was significantly thinner in those with higher HbA1c levels and/or diabetes, representing areas of the pRNFL that may be most affected by diabetes. Deep Learning to Detect OCT-Derived DME from Retinal PhotographsInvestigators validated the generalizability of a deep learning system (DLS) that detected diabetic macular edema from two-dimensional color fundus photography (CFP) in which the reference standard for retinal thickness and fluid presence was derived from three-dimensional optical coherence tomography images. The retrospective validation of a DLS across international datasets included paired CFP and OCT of patients from diabetic retinopathy screening programs or retina clinics. The DLS was developed using datasets from Thailand, the United Kingdom and the United States, and validated using 3,060 unique eyes from 1,582 patients across screening populations in Australia, India and Thailand. The DLS was separately validated in 698 eyes from 537 screened patients in the UK with mild DR and suspicion of DME based on CFP. The DLS was trained using DME labels from OCT. Presence of DME was based on retinal thickening or intraretinal fluid. The DLS's performance was compared to expert grades of maculopathy and to a previous proof-of-concept version of the DLS. Investigators further simulated integration of the DLS into an algorithm trained to detect DR from CFPs. Main outcome measures included superiority of specificity and non-inferiority of sensitivity of the DLS for the detection of center-involving DME, using device specific thresholds compared to experts. Investigators determined the DLS could generalize to multiple international populations with an accuracy exceeding that of experts. They suggested the clinical value of the DLS could reduce false positive referrals, thus decreasing the burden on specialist eye care, warrants prospective evaluation. Use of Intravitreal Triamcinolone Acetonide After PVDInvestigators evaluated the long-term outcomes of intravitreal triamcinolone acetonide administration after posterior vitreous detachment during pars plana vitrectomy for patients with proliferative diabetic retinopathy. A total of 189 eyes (152 patients) who underwent PPV for severe PDR were reviewed. Intravitreal injection of TA (IVTA) was administered during PPV in 118 eyes (PPV+IVTA group), and 71 eyes didn’t receive IVTA (PPV group). Immediately after PVD, when most of the vitreous and proliferative membranes were removed, 0.1 mL TA (40 mg/mL) was injected into the vitreous cavity in the PPV+IVTA group. All patients were followed for least 12 months. Visual outcomes and postoperative complications were recorded and compared between the two groups. Here are some of the findings: Investigators found administration of IVTA after PVD during PPV effectively improved the final visual outcomes of, and prevented postoperative complications in, patients with severe PDR. Br J Ophthalmol 2021; Nov 29. [Epub ahead of print].New Data from Beovu DME Study Novartis recently announced the first interpretable results from year two (week 100) of its Phase III KESTREL study. KESTREL assessed the safety and efficacy of Novartis’ Beovu (brolucizumab) 6 mg in patients with visual impairment due to diabetic macular edema. The company says that results from year two “confirmed the visual acuity gains, fluid reduction findings and safety profile from year one, while addressing the burden of frequent treatments for DME patients.” Around 40 percent of Beovu patients were maintained on 12-week dosing intervals, and 70 percent of patients who completed the first 12-week cycle after loading remained on 12-week dosing through year two, according to the company. Regenxbio Announces Initiation of Second RGX-314 Trial Regenxbio initiated ASCENT, the second of two Phase III trials, to evaluate the efficacy and safety of subretinal delivery of RGX-314 in patients with wet age-related macular degeneration. RGX-314 is being investigated as a potential one-time gene therapy for the treatment of wet AMD. A Biologics License Application is expected to be submitted to the FDA in 2024 based on ASCENT and the ongoing ATMOSPHERE trial, the company says. Read more.
Ribomic’s RBM-007 Fails to Demonstrate Improvement Over Eylea In the Phase II TOFU study of RBM-007 in patients with wet age-related macular degeneration, Ribomic announced RBM-007 monotherapy or RBM-007 in combination with Eylea didn’t demonstrate vision improvement over Eylea monotherapy in wet AMD. 4D Molecular Therapeutics Updates • 4D Molecular Therapeutics announced that the FDA granted Fast Track Designation to 4D-125 for treatment of patients with inherited retinal dystrophies due to defects in the RPGR gene, including X-linked retinitis pigmentosa. 4D-125 is designed to deliver a functional copy of the RPGR gene to photoreceptors in the retina. FDA Accepts Ocugen Investigational NDA for Gene Therapy Candidate Ocugen announced the FDA accepted the company’s Investigational New Drug application to initiate a human clinical trial of OCU400 (AAV-NR2E3), a modifier gene therapy candidate for the treatment of retinitis pigmentosa resulting from genetic mutations found in NR2E3 and rhodopsin.
